If you would like to schedule an appointment please complete our
online request form to the right or call our office. Also, if
you are a new patient to our office, for your convenience we have
provided our initial intake forms that you may print, fill out and
bring to your first appointment. Below is some other office
information to help you better understand how our office operates.
Office Hours:
8:00 AM - 5:00 PM Monday
-Thursday
8:00 AM - 3:00 PM Friday
Prescriptions &
Renewals:
Prescriptions and refills
are issued during office hours only. Please call our office to
request a refill before you run out of your current prescription.
We have up to a 48 hour turn around time on our prescription refills.
Appointments:
Please complete our online
request form or call the office to make an appointment in advance.
If you are unable to keep your appointment, please call us as far in
advance as possible in order for us to reschedule that time slot.
We make a sincere effort to adhere to our appointment schedule and
request that you arrive on time for your appointment as well.
Should you be more that 15 minutes late for your appointment, you may
be rescheduled. Also, if you have had diagnostic tests, such as
an MRI, please bring your films and report to your appointment.
If you arrive without the films and report, we may have to reschedule
your appointment. We are very sorry for this inconvenience and
understand that your time is very important, however, without this
information, it makes it extremely difficult for your physician to
diagnose your medical problem.
Fees & Payments:
We have set our fees with
the goal of being reasonable and fair, and have aligned them with the
relative value scale used by Medicare. In all cases, if you have
provided us with your insurance information, we bill your insurance
company prior to sending you a statement.
We accept cash, checks,
VISA, MasterCard, and Discover. If you do not have insurance, we
will offer a discount for office services if paid on the date of
service and discounts on surgery if paid in full within 30 days of the
procedure. For those of you with insurance, in most
cases we have already contracted with your insurance carrier for a
discount on our fees.
